I’ve spent over 20 years working as a Cardiologist, helping people manage conditions like high blood pressure, heart failure, and raised cholesterol. When it comes to managing weight, the reality is sobering: exercising a lot while eating whatever you like simply doesn’t work. It all comes down to priorities. Don’t get me wrong—I’m a huge advocate of regular exercise, and the research strongly supports it—but not as the main tool for weight loss. The real benefits of physical activity lie in improving metabolic health, building and preserving muscle, supporting mental wellbeing, and lowering the risk of conditions such as cancer, cardiovascular disease and dementia.
Every January, countless people commit to New Year’s resolutions, and many of those focus on healthier habits. I’ve always found it interesting that we wait for a symbolic date to start making changes that could help us feel better immediately, and for years to come. Perhaps most of us just need that extra nudge.

How Long Will I Need to Take Them?
Generally speaking, I expect most patients will remain on the medication for roughly a year. This gives us enough time to identify the most suitable dose (I always begin at a lower dose and increase gradually if required) and, importantly, to put sustainable lifestyle changes in place. These changes are key to ensuring the benefits last well beyond the point when the medication is discontinued.
I do have concerns about people remaining on these medications indefinitely. Ideally, I’d like patients to use the time they’re losing weight on the drug as an opportunity to establish healthy lifestyle habits that will serve them well in the long term. This approach means they can eventually come off the medication safely while continuing to enjoy the health and wellbeing benefits they’ve achieved.
For that reason, I work closely with anyone I prescribe these treatments for, supporting them in making those important lifestyle adjustments. This is something I’m genuinely passionate about—I live by these principles myself and believe in the power of small, consistent actions to maintain good health.
It’s simply human nature: think about how most of us step up our brushing and flossing when a dental appointment is looming. In the same way, I find that when patients know they’ll be checking in with me regularly, they tend to stay more focused on their health between visits.
What About Side Effects?
Like any medication, weight loss treatments can cause side effects, although serious reactions are thankfully rare. From what I’ve seen, the overall side effect profile is very reassuring. In clinical studies, the most commonly reported issues were nausea, vomiting and diarrhoea—but these symptoms only occasionally resulted in patients stopping the medication. If patients do experience side effects, I will work with them to help minimise any issues.
One important point to be aware of is that alongside fat loss, some muscle loss can occur. This is why I’m such a strong advocate of weight and resistance training for all my patients. I recommend it just as much to women in their 90s as I do to younger men—if anything, older patients have even more to gain from it! Whenever possible, I encourage those taking weight loss medications to incorporate resistance exercises into their routine to help maintain muscle mass.
